by Social Services is the key to our effort to move residents
through the difficult adjustment in long term care towards
achieving their maximum level of psychosocial functioning.
This workshop focuses on the four key areas of documentation
including social assessment, progress notes, care plans and
discharge plan and how to insure they are effective tools
in the interdisciplinary process.
