BACKGROUND
ON STAFF MEMBERS
Susan is a
licensed social worker with 18 years experience in the healthcare industry. Her
experience includes the nursing home setting, Acute Hospital discharge
planning, and Acute Rehab case management. She has been employed with Luken
Associates since 1996.
Mary Beth
has been employed in the healthcare industry for 25 years, with experience in
many aspects of long term care, and Acute Hospital discharge planning/case
management. She has been with Luken Associates since 1982.
Robin is an
LPN who has been in healthcare for 10 years. Her experience includes Home
Healthcare, Acute Hospital, Adult Daycare, and discharge planning/case
management in the long term care setting. She has been with Luken Associates
since 2000.
VANESSA
FLANNERY
Vanessa
has many years of experience with long term care starting in 1984. Her
duties have included admissions, marketing, social services and case management
with such prominent companies as SeniorCare, Kindred and Hurstbourne Care
Center. She has been with Luken Associates since 2003.
STAFF
RESPONSIBILITIES
Each staff
is assigned as a case manager for specific hospitals. All referrals from
these hospitals may be made by contacting the designated case manager.
Every effort is made to insure an onsite response within two(2) hours of the
referral. If you are seeking placement in long term care, or if you would like
more information about one of the facilities, please contact us. Our staff
routinely visits all the hospitals in the Louisville area as well as conduct
home visits.
